Our Partnership
PocketRN & the Medicare GUIDE Program
Ministering Hearts is proud to partner with PocketRN to bring families access to the Medicare GUIDE (Guiding an Improved Dementia Experience) program — a specialized Medicare benefit designed to support people living with dementia and their caregivers.
What Is the GUIDE Program?
The GUIDE program is a Medicare initiative focused on improving the quality of life for individuals living with dementia and reducing caregiver burden. Through this program, eligible participants can access a coordinated care team, caregiver education and support, and respite services.
Program Eligibility
Eligibility for the GUIDE program is determined by Medicare and your care team. We encourage families to contact us to learn whether your loved one may qualify and to discuss how the program could benefit your specific situation. We do not make eligibility determinations — our team will connect you with the right resources to find out.
Caregiver Support
Family caregivers are at the heart of dementia care. The GUIDE program and our partnership with PocketRN are designed to support not just the person living with dementia, but also the family members and caregivers who provide daily care. Support may include education, coaching, and access to respite care.
